We are experiencing significant growth and diversification in population in Edgewater.
The addition of many outdoor activities and with the community being named the first designated Blueway Community in Volusia County and Edgewater's Rails to Trails trail opening in early November which will travel from Edgewater all the way to lake Monroe in Sanford.

Jeffrey R. Berner
Jeffery R. Berner initiated his education at Penn State University and completed his master's degree at Barry University in Miami.
Jeff has been a member of the American Physical Therapy Association since entering the profession in 1988 and has served as chairperson for the East Central District of Florida.
He has attended multiple continuing education courses to advance his clinical skills in diverse areas of practice, and he is an adjunct faculty member at Daytona Beach Community College in the Physical Therapy Assistant program.

David Antolick
David Antolick was born in upstate New York and moved to West Palm Beach, Florida, in 1993.
David, his wife and two children relocated to Central Florida and now reside in Port Orange.
In 2002, he earned his Associate of Science degree in Physical Therapist Assisting at South University in West Palm Beach.
He is a member of the Florida and American Physical Therapy Associations.
David looks forward to helping you achieve your rehabilitation goals while exceeding your physical therapy expectations.

Michelle Applegate
Originally from Oregon, graduated from Oregon State University in 1992 with a BA / BS in Biology.
From there, she landed in Hawaii where she received her Physical Therapy Assistant degree in 1994 and her Masters in KLS - Athletic Training in 2001.
During that time, she had opportunity to work with various University of Hawaii athletic teams along side the sports medicine physicians for 3 years.
Michelle received her Doctorate in Physical Therapy from the University of St. Augustine for Health Sciences in 2004 where she specialized in Manual Therapy.
From there, she landed in Hawaii where she received her Physical Therapy Assistant degree in 1994 and her Masters in KLS - Athletic Training in 2001.
During that time, she had opportunity to work with various University of Hawaii athletic teams along side the sports medicine physicians for 3 years.

There are two aspects to consider 1) Legal need for prescription and 2) Insurance requirement.
The State of Florida license medical practitioners allowing them to practice their profession in this state.
FS486 states that If physical therapy treatment for a patient is required beyond 21 days for a condition not previously assessed by a practitioner of record, the physical therapist shall obtain a practitioner of record who will review and sign the plan.
A health care practitioner licensed under chapter 458, chapter 459, chapter 460, chapter 461, or chapter 466 and engaged in active practice is eligible to serve as a practitioner of record.
